Medication Management: Review and verify medication orders for
accuracy, appropriateness, dosage, and potential interactions.
Clinical Consultation: Work directly with physicians, nurses, and
other healthcare professionals to provide expertise on medication
therapy management and advise on drug selection, dosing, and
administration.
Patient Safety: Monitor patients for adverse drug reactions, drug
interactions, and allergies, ensuring that treatments align with
evidence-based guidelines.
Pharmacokinetic Monitoring: Monitor and adjust medications based on
pharmacokinetic parameters (e.g., for antibiotics, anticoagulants,
and other high-risk medications).
Medication Therapy Review (MTM): Conduct medication reconciliation
during patient admission, transfer, and discharge to ensure the
accuracy and continuity of medication orders.
Education: Educate patients and hospital staff on the proper use of
medications, including any potential side effects, administration
techniques, or precautions.
Inventory Control: Ensure proper storage, dispensing, and tracking
of medications in the hospital's pharmacy.
Emergency Response: Provide pharmacy support during medical
emergencies, including code situations, and assist in the
preparation and dispensing of emergency medications.
Documentation: Maintain accurate patient records, documenting
consultations, medication therapy changes, and clinical
recommendations.
Additional Duties:
Research & Protocol Development: Participate in research or quality
improvement projects related to medication management or pharmacy
practice.
Regulatory Compliance: Ensure compliance with hospital policies,
state and federal regulations, and accreditation standards
regarding pharmacy practices.
Supervision: Supervise pharmacy technicians, interns, or other
pharmacy staff as needed.
Qualifications:
Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited
school of pharmacy.
Licensing: Active pharmacist license in the state of practice.
Experience: Previous experience in inpatient pharmacy practice or
clinical pharmacy preferred, though new graduates may be
considered.
Knowledge: Strong understanding of pharmacology, therapeutics, and
hospital pharmacy operations.
Skills: Excellent communication skills, attention to detail,
ability to work under pressure, and proficiency in clinical
decision-making.
Certifications: Board certification in pharmacotherapy (BCPS) or
other specialty certifications may be required or preferred.
