Job SummaryWe are seeking a dedicated and compassionate Hospice Certified Home Health Aide to join our team at Interim HealthCare Hospice. As a Hospice CHHA, you will play a crucial role in delivering exceptional end-of-life care to our patients and their families.Key Responsibilities:

• Provide personal care services to patients in their homes, including bathing, toileting, dressing, and grooming.
• Assist with daily activities such as eating, nutritional intake, and vital sign monitoring.
• Work collaboratively with our hospice nurses to ensure a patient's care plan is followed.
• Observe patient conditions and notify supervisors of any changes requiring a change in care, medication, or medical equipment.Requirements:
  • Active Home Health Aide certification and active Certified Nursing Assistant (CNA) Certification in California.
  • CPR certification.
  • Covid-19 vaccination card.
  • Valid California driver's license, auto insurance, and transportation.
